How does a fixed IP SIM card work?

A fixed (static) IP data SIM is allocated it’s fixed IP address externally to the main network operator. Typically, the SIM will be directed to a private APN where the SIM traffic is passed onwards into a private carrier network, such as an ISP; this is where the fixed IP address then becomes fixed to the SIM’s identity and allows both inbound and outbound connectivity across the internet.
